A provider should understand that while taking care of a patient is of the highest priority, documentation for services provided is inclusive to the patients overall care.

Level of specificity is one of many reasons that delay or deny payments. ICD-10 is going to play a significant role in the Level of Specificity and laterality (Left vs Right). Documentation will require more specificity in terms of site (location), Trimester vs weeks (OBGYN), etiologies of conditions, when, where and how an injury/event occurred. Also, the status of a patient (Civilian vs Military).

With that said, How should a physician keep his/her documentation in good standing?

Are Saturated Fats the Cause of Heart Disease?

People suffering from type 2 diabetes are constantly being told by their doctors and dietary nutritionists alike that saturated fats are the cause of heart disease. Doctors tell them to stay away from anything containing this ingredient. They are being told this because they have a greater chance of suffering from heart disease. The truth is, those type of fats are NOT the cause of cardiovascular disease. There are some facts to prove that it’s not the saturated fats that’s causing this killer disease, but it’s the packaged, processed, sugar, and refined foods that everyone is consuming that is really causing an alarmingly high percentage rate of heart disease in our country currently.

The fact of the matter is before 1920 incidents of cardiovascular disease was extremely low and, it was not until about 1950 did we see this increased rate in cardiovascular disease. She writes, “During the sixty year period from 1910 to 1970 the proportion of traditional animal fat in the American diet declined from 83% to 62%.” You mean to tell me that the consumption of saturated fat decreased? “And butter consumption plummeted from 18 pounds per person each year to four pounds per person each year.” So were people actually eating a significantly less amount of butter during that time? “During the past eighty years the consumption of dietary cholesterol intake has increased by only 1%.” Question…if this is true and people are actually eating less saturated fat, eating less butter, and they’re not eating very much more cholesterol, how could that be the cause of this increase in heart disease?

Well, Sally Fallon basically answers that question in her book. “During the same period the average intake of dietary vegetable oils in the form of margarine, shortening, and refined oils increased by 400%.” That is astounding! Once we started eating these refined oils, the incidence of heart disease drastically increased. She also says, “The consumption of sugar and processed foods increased by 60%.” And this is not even all of the research.

People are eating way too many processed, fake foods, loads of processed carbohydrates, tons of sugar, and since I decreased these foods from my own diet, I have become much healthier. So taken these fats into account… It’s not the saturated fat, It’s not the healthy, organic raw butter that you can cook with that’s causing heart disease. It’s also not the unrefined, organic coconut oil, or the natural grass fed beef. These things are fine. And it’s also not the egg yolks causing high cholesterol. It’s all these PROCESSED foods, such as cakes, cookies, loads of pasta, and tons of bread that’s causing the incidence of disease. Sherlock Holmes and the Modern Affair

The second Guy Ritchie directed Sherlock Holmes movie doesn’t hit cinema screens until December, but Warner Brothers have already confirmed that there will be a third movie from the same team. Warner Bros has set Drew Pearce to write Sherlock Holmes 3, continuing the Guy Ritchie-directed series that stars Robert Downey Jr. and Jude Law.

The first in the Guy Ritchie Holmes series was a huge success and seemed to please most film fans, but it received mixed reactions from traditionalist Holmes fans who see this new Holmes as too far from Doyle’s original character. I enjoyed the movie myself, but I did feel that it more resembled a steam punk influenced Victorian action movie than a Holmes adventure. However the studio, at least, didn’t bow down to political correctness and it was refreshing to see Holmes with his trademark pipe, unlike the BBC series, Sherlock which has the detective on nicotine patches. I applaud the makers of the movie in keeping the pipe, especially in these days of tobacco hysteria. At least they didn’t make the mistake of the current producers of the James Bond movies. They’ve taken away Bond’s dick and look what’s happened to the world’s favourite spy! James Bland, anyone! The current screen Bond is indistinguishable from any other screen hero, but at least Holmes in all his incarnations has managed to retain something of the unique character which made him such a success in the first place.

Because the BBC series is superbly entertaining and the pairing of Benedict Cumberbatch as Sherlock Holmes and Martin Freeman as Watson, works well, that the series is set in contemporary times doesn’t really matter – after all several of the Basil Rathbone Holmes movies were contemporary. But at the end of the day the series isn’t really Sherlock Holmes, just as Guy Ritchie’s movies, entertaining in themselves, are not really Sherlock Holmes.

Holmes though continues to go on and on and as well as the new movie this December and a second series of Sherlock early next year, the great detective is kept alive in print by new authors taking up the pen left by Doyle and continuing to add to the remarkable evergreen character. Holmes has been presented at one time or another as a boy, a woman and even a dog, fox and mouse. As well as everything else between – I once read a short story in which Sherlock Holmes was a motor car and his assistant was the loyal, Datsun.

So does it really matter how characters like Sherlock Holmes are presented? The variations on the characters and themes may not please the purists, but they might just encourage new readers to try the original canon – and that can only be a good thing.

“There is still more excitement and invention in the original tales than a thousand screen incarnations.”

No matter how many actors, how many writers tamper with the creation, Holmes will never die and will always be out there, wearing that deerstalker and puffing on the pipe. The detective is a hard man to kill as even original creator, Conan Doyle discovered when he tried to hurl the detective over those waterfalls that I can’t be bothered to Google the correct spelling off.

Stuttering In Children

Stuttering is a problem that affects three million Americans. Most people begin to stutter when they are children. Stuttering in children will begin to stutter between the ages of three and five years old. For many children, stuttering will be a very short-lived thing. Children will often stutter as they begin to develop their language skills. Most children will just grow out of it after a short season of stuttering. It is estimated that between five and seven percent of children will stutter when they are developing their language skills. However, 1% or less of adults struggle with stuttering.

Stuttering in children will most likely begin when the child is between 18 and 24 months old. It will start out with them having a harder time putting sentences together. For most children these struggles will be short-lived and will only last for a few weeks and at most several months. Children will have mostly outgrown stuttering by the time that they are five years old. If a child continues to have trouble speaking after several months, then a parent should consider seeking out a speech pathologist.

Most parents are encouraged to not try to interrupt their child or to try to help their child out when they are going through this phase. Parents should be very supportive of their children at this time in their lives and not get upset if their child begins to stutter on a few words.

A parent should not have his child repeat words or try to have him slow down when the child is talking. The parent’s job should be to let the child work out the stuttering for himself. The more the parent tries to correct the stuttering or bring a lot of attention to the stuttering the greater the risk that the child will of continue to struggle with stuttering when he needs not to. Stuttering in children will affect twice as many boys and girls. Girls are less apt to go through a stage early in life when they struggle with stuttering.

Many children who are at a greater risk of continued stuttering after they face the typical period in life when most children stutter are those that have a family history of stuttering. Other factors of children who continue to stutter after this time in their lives is that they will be highly sensitive and could have some very strong and pronounced traits of a perfectionist. The Six Types of Hypertension

Most people think that there is only one type of hypertension. They usually refer to it as high blood pressure. It has been called the “silent killer”. That’s because there usually are no symptoms to alert the sufferer. It is a good idea to have your blood pressure checked regularly.

The common type is known as primary hypertension sometimes called essential. Even though it is the most diagnosed type, there is no known cause.

Next, there is the type called secondary hypertension. It is usually caused by factors that are sometimes curable. Some of the causes are pregnancy related. It can also be brought on by thyroid dysfunction, tumors, sleep apnea, alcohol, and some medications to just name a few.

Malignant Hypertension is the worst type. It is very severe and it just keeps getting worse with time. It very quickly will lead to organ damage. If it is not properly treated, it will result in death in as little as five years for most sufferers. Intensive treatment can reverse the condition and prevent complications. It is not brought on by cancer. And, it is not caused by any malignancy.

Isolated Systolic type may be diagnosed in older folks. The systolic, which is the top number, stays above 160. And, the diastolic is below 90. This is caused by the stiffening of the arteries. And the loss of the elasticity in arteries like the aorta; sometimes diagnosed as arteriosclerosis. It is very important to receive treatment, because it can help prevent strokes and heart disease.

White coat type is brought on by anxiety. The individual’s blood pressure is high only when it gets tested by someone in a clinical environment, like a hospital. Here is where the term “white coat” comes from. It is then recommended that such a person have blood pressure readings performed in a more relaxed area. If it shows a normal reading, then simple follow up tests in a non clinical area will be sufficient. It is good to make sure that persistent hypertension is not developing.

Make a few lifestyle changes. Less alcohol and cut down on the salt. Maybe lose some weight. Eat a low fat diet. Have more fruit and green vegetables.
